    As home health agencies learn how to achieve the best outcome at the lowest cost, we find best practice PDGM leverages the skills of each clinical discipline and office-based care managers. During the shift to PDGM, therapy utilization came under intense scrutiny, despite the role of therapy remaining unchanged. What has changed is the manner in which the interdisciplinary team approaches care planning, which yields solid clinical and financial outcomes. Join us as we look through the lens of therapists, nurses, and care managers, providing key guidance for planning a 30-day PDGM payment period and achieving best practice utilization management.

    Objectives:

    • Describe a best practice care management approach to interdisciplinary utilization management throughout the 30 day payment period and 60 day episode of care.

    • Integrate practice guidance as a therapist or nurse in planning PDGM-friendly care delivery

    • Identify common pitfalls when planning care, leading to missed payment periods and increased LUPA potential.

    BLIND SPOTS OF DIVERSITY, INCLUSION & IMPLICIT BIAS

    Diversity measures our ability to share differences, build relationships, and be creative thinkers. Inclusion measures our ability to embrace these differences through participation. Implicit bias limits our ability to be successful in diversity and inclusion efforts without even knowing it. Understanding the dynamics of diversity and inclusion will help shape our ability to see the blind spots of implicit bias that exists within. While training and mentorship expedite professional development, engaging diverse ideas, perspectives, and insight leads to improved customer experience, faster innovation, and greater retention rates. This is true when implicit bias does not block the view. The world is on the cusp of change and the future requires leaders willing to see beyond the surface.

    ADDRESSING HEALTH EQUITY IN THE NEW NORMAL
    Allina Health used actionable data to identify potential areas of bias, then applied the right interventions to decrease implicit biases. For example, data revealed that the African American populations receiving care at Allina Health were not enrolling in hospice programs when they were eligible because the hospitalists weren’t referring African Americans at the same rate as other populations. Join Vivian Anugwom, Health Equity Manager at Allina Health, to learn how she led a team to implement new measures, including implicit bias trainings, to help address and overcome these biases to ensure health equity for all.

    OASIS data collection continues to evolve! Based on the most current CMS guidance available, this workshop provides a comprehensive look at the OASIS items including the OASIS-D1 revisions and OASIS related PDGM information effective January 1, 2020.  This effective, up-to-the-minute education is targeted for field data collectors, their supervisors, OASIS educators and reviewers, quality improvement staff, and those preparing for the COS-C Exam.

    Experience the comprehensive and nationally acclaimed two-day Blueprint for OASIS Accuracy workshop and learn to confidently teach, audit and collect OASIS-D1 items accurately. Through guided, relatable, expert instruction, and participation in problem-solving discussions and more than 50 application scenarios, achieve mastery of the OASIS items, conventions, and the latest data collection rules.

    The workshop educational objectives include:

    • Selecting OASIS responses that accurately represent clinical assessment findings
    • Providing accurate data for quality measurement and payment
    • Documenting OASIS data collection compliant with CMS resources and instructions

    Rather than provide opinions, assumptions, or unfounded interpretations, the “Blueprint” presenters will model reliance on CMS guidance documents and provide and demonstrate strategies for how to find defendable answers to your OASIS questions.
